For referring professionals

A considered connection between care teams and families.

Start with the reason for the referral, the people involved and the information that can appropriately be shared. Keep clinical decisions with the responsible care team and use the approved process for any patient records.

What matters here

Purpose and permission

Confirm the purpose of the referral and the basis for sharing before transferring records.

The right participants

Identify the referring professional, the family or representative, and the person coordinating the next step.

A private handoff

Use only the authorized workflow enabled for your workspace. Public pages and ordinary contact channels are not a patient-document drop.

A practical path

Give each step the attention it needs.

Keep confirmed details and unanswered questions separate as you move forward.

  1. Prepare the conversation

Clarify the proposed timing, the questions for placement guidance and the receiving team's contact arrangement.

  2. Confirm the sharing process

Check who is authorized to send and receive each part of the information. An account alone does not grant case access.

  3. Follow the authorized next steps

Use your invited Referrer workspace for information shared with your account. Ask the coordinating advisor about a new referral if no submission workflow is available.

Before your conversation

A few things to have in mind.

You can use this list without entering or uploading personal information.

  • Referring professional and organization
  • Who is coordinating with the family
  • Purpose and permission for sharing
  • The approved private workflow for records

Common questions

A little more context.

Is there an anonymous patient upload on this website?

No. Patient records must follow the authenticated, authorized workflow provided for the workspace. Do not place them in a public page or an unapproved channel.

Can I browse every case after signing in?

No. A referring account sees only information specifically shared with it. A submission or invitation does not confer agency-wide access.
