For families

A personal decision. A thoughtful place to begin.

A change in living arrangements touches more than an address. Begin with the person's daily life, the people they want involved, and the questions that need an answer before choosing a home.

What matters here

The person at the center

Favorite routines, relationships, food, language and room preferences belong alongside practical support needs.

A shared understanding

Agree on who will join the conversation, who can make decisions and which information each person may receive.

A manageable next step

Turn a broad search into a short set of questions for the advisor, the care team and each home under consideration.

A practical path

Give each step the attention it needs.

Keep confirmed details and unanswered questions separate as you move forward.

  1. Name what matters

Start with the person's preferences and the changes prompting the conversation. Separate essential needs from flexible preferences.

  2. Gather the right information

Ask the care team what a prospective home needs to understand. Use the approved private sharing process for health records.

  3. Compare and revisit

Review options with the people involved, visit when possible and return to questions that remain unanswered.

Before your conversation

A few things to have in mind.

You can use this list without entering or uploading personal information.

  • The routines and relationships the person wants to keep
  • Who should participate in discussions and decisions
  • Questions about daily and overnight support
  • A practical budget range and preferred timing

Common questions

A little more context.

Do we need to know which home we want?

No. Start with the person and the questions you have. The care-consideration guide helps organize an initial conversation without choosing a home first.

Can every family member see the same records?

Access follows each person's approved role and disclosure permissions. A family relationship or shared email does not automatically authorize access to health or financial information.
